How we protect your health information.

Our commitment

This notice describes how medical information about you may be used and disclosed, and how you can get access to that information. National Pain Centers (“NPC”) is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we use your information

We may use and disclose your health information, without additional authorization, for treatment, payment, and health care operations — and to remind you about appointments. We never sell your health information, and we do not use it for marketing without your written authorization.

In limited circumstances permitted or required by law, we may disclose information without your authorization — for example, for public-health activities, reporting suspected abuse, health oversight, judicial proceedings, or to avert a serious threat to safety.

Your privacy rights

You have the right to inspect and obtain a copy of your records, request a correction, request an accounting of certain disclosures, ask us to restrict how we use or share your information, request confidential communications, and receive a paper copy of this notice at any time.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint. Contact our Privacy Officer at privacy@nationalpain.com or (888) 555-0117.
