Every member of the National Pain Centers team was personally and deliberately selected: not only for experience and credentials, but for the things a résumé can’t show — compassion, character, ethics, and a refusal to accept “good enough.”

The physician who sets the bar
Dr. Joshi is a board-certified, anesthesiology-trained, accredited-fellowship-trained interventional pain physician. In 1999 he became the first U.S. pain physician to work with the World Health Organization’s Department of Substance Abuse in Geneva — and he warned of the OxyContin epidemic more than a decade before it was widely acknowledged. In three decades of practice, he has never prescribed OxyContin or oxycodone.
He is also one of the only interventional pain physicians to have studied Ayurvedic medicine — drawing not on its treatments but on its whole-person philosophy: understanding each patient’s individual constitution and favoring natural, safe options wherever they truly serve. A published co-architect of the landmark stellate-ganglion-block unifying theory and the developer of pioneering procedural techniques, Dr. Joshi built this practice — and hand-selected this team — around a single idea: category-of-one care, obsessively pursued, at every level.

Grant Reffell, DC, is a licensed chiropractor who brings decades of leadership, hands-on experience, and a track record of strong outcomes in musculoskeletal, therapy, and chiropractic care. His work is central to our whole-person, multi-modal approach — addressing how the body moves and recovers, not just where it hurts.
Deanne Giannola, PA-C, brings decades of experience in patient care — including surgical and cancer care — along with the meticulous organization that keeps complex care running smoothly. Patients feel the difference in how thoroughly, and how personally, they are looked after.
Dr. Desai is double board-certified in internal medicine and obesity medicine, consulting on obesity and lifestyle medicine, GLP-1 therapy, and nutrition. An obesity-medicine expert, she has served as an early-stage lead physician at multiple obesity-medicine companies — bringing both clinical depth and a builder’s view of where metabolic care is heading. She anchors our weight-management and metabolic-optimization program.
Kendra leads operations at our surgery center, orchestrating the people, workflows, and standards that let complex procedures run safely and seamlessly. When a day at the center runs smoothly, it is usually because of the systems she has built behind the scenes.
Paulina directs nursing at our surgery center, setting the standard for the clinical care, safety, and compassion patients experience before, during, and after every procedure.
Kayla is part of the nursing team that keeps every procedure safe, comfortable, and closely monitored — from preparation through recovery — with a steady, reassuring presence patients remember.
Melissa brings skilled, attentive nursing care to the surgery center, watching over patients at every step so comfort and safety are never an afterthought.

Nirjor develops AI-driven healthcare and computer-vision solutions, deploying computer-vision and large-language-model applications for real-world use, and designs scalable systems that bring machine learning and LLMs into everyday care.
Meraj builds healthcare Android apps with Kotlin and Jetpack Compose — including on-device AI for body-point detection and movement tracking — alongside responsive React and Next.js frontends with speech recognition, chatbots, and secure APIs.
Desha builds scalable, responsive web applications with Next.js, React, and TypeScript, backed by full-stack Node.js work — contributing to AI-powered healthcare solutions and real-time, secure web experiences.
Sadi anchors research — experiment design, results analysis, and academic writing — with research-associate experience at the University of Tsukuba and the University of Rajshahi, and is a Monbukagakusho Honors scholar.
Krista guides members and patients through the financial side of care with clarity and patience — coverage, estimates, and paperwork handled, so you can focus on getting better rather than decoding a bill.
Adriana is often the first voice patients hear and the first face they see. From scheduling and check-in to answering the questions that come up along the way, she does a little of everything — and makes sure every patient feels genuinely welcomed and looked after.
